I'm really frustrated,I keep getting bad grades on my tests and I try so hard but fail every time. Please help? - which side do you write on blank card

So here's my problem:

I do all my homework. I never missed an assignment in my life. Although when it comes to testing, it is missing almost impossible for me not to.

I added, but I'm too medical (with Adderall XR). Whenever I start studying, I still distracted side, which is one reason for the failure. Although, if you fall behind, and discover for hours, still not the test!

I hear all the facts and remember them, but when I read the first question blank in my head out and forget everything, what I learned. I find myself reading all the questions again and again and drop the words, but not in my head and I do not even remember what the problem was long enough for the automaticand the answer below!

I vowed that if he started this school year much more difficult to concentrate and to remember the questions and not nervous, but I can not! I have very bad in two of four tests that we have done so far. I am always disappointed to have a poor-house newsletter, and constantly stress about it.
